		Annex Delivery for UB Community - Book UB ONLY - Use this form if you are a current UB student, faculty or staff member and after consulting the BISON Catalog, you have determined that a book you want is held in the Libraries Annex.

		Annex Delivery for UB Community - Journal Article or Book Chapter UB ONLY - Use this form if you are a current UB student, faculty or staff member and after consulting the BISON Catalog, you have determined that an article or book chapter you want is held in the Libraries Annex.

		Annex Delivery for non-UB patrons Use this form if you are NOT a current UB student, faculty or staff member and after consulting the BISON Catalog, you have determined the material you want is held in the Libraries Annex.

		Assignment Alert This form is for all University at Buffalo faculty and teaching assistants to inform University librarians of current research assignments which require library use.

		BISON Catalog Problem Use this form to notify the Catalog Department of recommended corrections or modifications to the BISON Catalog.

		Contact Us We encourage you to send us your comments and concerns regarding the University at Buffalo Libraries web site or resources.

		Course Reserve Submission You are an instructor and want to put books, articles & other materials on reserve for your students.

		Disability Services You are registered with the UB Office of Disability Services and need article photocopying or book retrieval.

		Electronic Journal Problem Report Form Please complete the form below to report any technical difficulties that you experience when accessing electronic journals.

		Electronic Journal Request Form You want to recommend that the UB Libraries purchase an electronic journal.

		General Catalog Services To request items noted in the BISON Catalog as "In Process," "On Order," "Incomplete Issues," " At Bindery," or "In Preservation".

		In Storage To request items noted in the BISON Catalog as "In Storage," "Storage Item Ask at HSL Circulation Desk," "Arch/Plan Storage," or "In Storage Ask at Music Library".

		Interlibrary Loan UB ONLY - To request a book, journal article, or other item that is not listed in the BISON Catalog.

		Library Instruction - General - Graduate Level You are an instructor and want to request a library instruction session for a graduate classes in all relevant academic areas.

		Library Instruction - General - Undergraduate Level You are an instructor and want to request a library instruction session for an undergraduate classes in all relevant academic areas.

		Library Instruction for High School Teachers & Librarians You are a high school teacher or librarian and want to request a library instruction session.

		Library Instruction - Health Sciences Library To request a course-related instruction session.

		Online Resources Technical Problems To report any technical difficulties that you experience when accessing library online resources.

		Purchase Recommendation You want to recommend that the UB Libraries purchase a book or other material.

		Reference Question You are a member of the UB community (or have a special interest in our collections) and want to ask a question.

		Research Consultation You are a member of the UB community and want to ask a make a research consultation appointment.

		Special Collections To request access to items in Special Collections, (Poetry, Archives, or Rare Books).

		Study Carrel Application for Lockwood Library You are faculty, professional staff, Ph.D. candidates and graduate students from the College of Arts & Sciences, Graduate School of Education, and the Schools of Architecture and Planning, Management or Social Work and would like to apply for a study carrel in the Lockwood library

		UB Document Express UB ONLY - You are on the North or South campus and the item you need is listed in the BISON Catalog as being on the other campus.

		UB Document Express for Roswell You are at Roswell Park (RPCI) and you have determined that a journal article you want is held in a UB library.

		UB Institutional Repository Project Request Form Request projects to be included in the University at Buffalo Institutional Repository.
